- Completed Registration Form
- $115. check for registration fee made payable to LVF Cheerleading Fund
- $100. check dated 12/1/10 for work bond made payable to LVF Cheerleading Fund (refund will given upon work bond completion)
- Proof of residency (i.e., tax bill, utility bill)
- COPY of Birth Certificate
